FOR PATIENTS WITH INSURANCE COVERAGE FOR FERTILITY TREATMENT:
Please note that Arizona Reproductive Laboratories, LLC (AARL) in not contracted with most insurance companies. Payment for services rendered by AARL (including andrology, reproductive hormone testing, and embryology) will be due at or prior to the start of treatment. At the conclusion of your IVF cycle itemized receipts will be provided to you so that you may submit a claim to your insurance company for reimbursement.
