Below you will find a list of insurances that Advanced Fertility Care is currently contracted with. Once you schedule your initial appointment with us, we will verify benefits with your insurance company but it is important that you become acquainted with your coverage and any procedures that are required (such as referrals and prior authorizations).
After you have spoken with your insurance company or your HR department, please feel free to call our business office if you have additional questions.

If you find that you must cancel your appointment based on your lack of coverage please call the office to cancel the appointment. Failure to cancel your appointment without a 24 hour notice will result in a missed appointment fee.
Please note that Arizona Advanced Reproductive Laboratory, LLC (AARL) is not contracted with most insurance companies. Payment for services rendered by AARL (including andrology, reproductive hormone testing, and embryology) will be collected at time of service or invoiced as appropriate.
