Aetna Products
Aetna insurance requires patients seeking infertility care to pre-register with the Aetna Infertility Line at 1-800-575-5999.  Aetna personnel will advise the patient whether or not registration or authorization codes are necessary based on the patient's individual plan requirements.   

Patients are asked to document the details provided to them during this call (Date, Time, Aetna representative name, and any additional specifics) and relay this information to Advanced Fertility Care at the time of their initial visit.

Failure to pre-enroll with Aetna may result in a denial of coverage from Aetna.   If Aetna were to deny coverage the patient would be responsible for payment in full at the time of visit.

Arizona Foundation
All types. Every plan is different and the covered benefits are mandated by the individual employers.

Banner Healthcare (effective January 1, 2011)                                                                                                 Banner coverage may differ from one individual to another.  Please refer to your individual insurance plan and stipulations.  Contact Banner Health Human Resources for more information.

Blue Cross / Blue Shield (in and out of state) types.

Beech Street

CIGNA
All types. Every plan is different and the covered benefits are mandated by the individual employers

Golden Rule
While we are contracted, they do not cover any infertility testing or treatment. They also only cover a portion of maternity charges.

Humana
nearly all of the Humana Plans exclude all infertility services. Please check in your certificate of coverage in the exclusions sections for details.

Tricare
Tricare requires an authorization from your PCM prior to scheduling your appointment. Tricare covers diagnostic services only.

United Healthcare
All types. Every plan is different and the covered benefits are mandated by the individual employers.

We will verify benefits, as well, with your insurance company but it is important that you become acquainted with your coverage and any procedures that are required (such as referrals and prior authorizations). After you have spoken with your insurance company and/or your HR department, please feel free to call the business office if you have additional questions.

If you are in need of patient financing, we have several companies on our website that can assist with this process.
If you find that you must cancel your appointment based on your lack of coverage please call the office to cancel the appointment. Failure to cancel your appointment without a 24 hour notice will result in a missed appointment fee.
We have found that being well informed about your insurance coverage will empower you as you proceed through your treatment. We appreciate you as patients and we are here to help in any way we can.

Please also be advised that ArizonaReproductive Laboratory is NOT contracted with any of the insurance plans. All services performed by this lab will be self pay at time of service. We will be happy to provide you with a detailed receipt for you to submit to your plan for any out of network reimbursement. Please see the AARL financial agreement for more details.
