Who Is Eligible For the Package Plans?

To qualify for the plan, both partners must undergo a medical screening to make sure you are good candidates for success with IVF. The specific requirements for eligibility will be reviewed with you by Dr. Zoneraich or Dr. Larsen during your consultation.

* For patients with insurance coverage for Fertility, please note that Arizona Reproductive Laboratories, LLC (AARL) does not contract with any insurance companies. Payment for services rendered by AARL (including andrology, reproductive hormone testing, and embryology) will be due at or prior to the start of treatment. An itemized receipt will be provided for you to submit to your insurance company for reimbursement.
