In order to qualify under our Donor Egg IVF Program a woman must be able to complete her treatment cycle and embryo transfer prior to turning age 50.

If you have > 12 Mature Eggs, you will receive a $4,000 refund

If you have < 12 Mature Eggs, your cycle reverts to a dedicated cycle.  No refund is given.

* For patients with insurance coverage for Fertility, please note that Arizona Reproductive Laboratories, LLC (AARL) does not contract with any insurance companies. Payment for services rendered by AARL (including andrology, reproductive hormone testing, and embryology) will be due at or prior to the start of treatment. A receipt will be provided for you to submit to your insurance company for reimbursement.
