Initial Consultation
When you have an initial consultation with Dr. Zoneraich, plan to spend between 45 to 60 minutes reviewing all the information relevant to your infertility. This will include a comprehensive evaluation of your previous care, your history and medical records. During this visit, you will have all of your questions answered, and you will participate in developing a plan for your testing and treatment. At Advanced Fertility Care, if there are going to be 2 parents involved in the fertility process, we strongly encourage both partners to be present at the initial consultation. Dr. Zoneraich will offer a plan of care which will include appropriate diagnostic testing as well as an explanation of potential options for treatment as it relates to your individual situation. Before you leave the office, you will have a clear understanding of your next steps.
In order for Dr. Zoneraich to prepare for your visit, it is important for him to receive your medical history before you arrive. Dr. Zoneriach will read all of the information you provide in advance so that he is able to study your individual situation and have a meaningful conversation with you at your initial consultation.
Appointment Checklist
Please make arrangements to complete the following steps BEFORE your first appointment
- After scheduling your appointment, pre register on line using “patient portal”
- Download and complete your new patient forms; patient registration, medical history questionnaires (both female and male), authorization and consent forms.
- Complete a written records release form and obtain a copy of your medical records from your OB/GYN, primary care physician, or another fertility program. Also include medical records from the Urologist, if applicable.
- Obtain copies of any prior HSG (hysterosalpingogram) reports and films from the hospital, radiology laboratory, or another fertility program, if applicable.
- Insurance cards and drivers licenses, for both partners.
- A referral from your primary care physician, if applicable.
- Insurance prior authorizations, if applicable.
BRING the following items with you to your first appointment
- Any referrals or prior authorizations, if applicable.
- Insurance cards and drivers licenses, for both partners.
- Completed new patient forms (medical questionnaires, registration forms, consent forms, authorization forms, etc…)
- Medical records from your OB/GYN, primary care physician, another fertility program, or the Urologist.
- Prior HSG (hysterosalpingogram) report and film from the hospital, radiology laboratory, or another fertility program, if applicable.
- Form of payment (we accept cash, personal checks, VISA, MasterCard, and Discover).
